The information we collect about our customers and their employees and provide to nonaffiliated third parties is for reasons such as premium processing, claims processing, reinsurance, reporting, investigating, or preventing fraud or material mis-representation, evaluating requests for coverage, other administrative functions, agency operations, activities and evaluations, and as otherwise required or permitted by law.

Confidentiality of Health Information

It is often necessary for CCAB to obtain your personal health information in order to facilitate the underwriting of insurance and to process claims. Our contractual relationships with health care insurance insurers, as well as state and federal laws, require that we keep your medical information confidential. We will not disclose your health or medical information to anyone without your authorization unless the law permits or requires us to do so.

Safeguarding Information

CCAB has physical, electronic and procedural safeguards in place to protect your nonpublic personal information. Only CCAB employees who are trained in the proper handling of your nonpublic personal information and who must have that information to perform their jobs will be allowed to see and use your information. Individuals who violate these policies and safeguards are subject to disciplinary action.
